Indiana Gastroenterology, Inc. (IGI) welcomes new patients for consultation. You do not need a physician referral unless required by your insurance carrier. Every attempt is made to schedule at your convenience however our availability may be limited by location, in-patient activity or physician absence. If you are not satisfied with your appointment date, we will place you on our waiting list at your request.

We respect your time and strive to run on schedule. Each patient is unique and time spent in consultation may vary. Please be patient with us, but let us know if you are kept waiting more than 15 minutes past your appointment.

We ask that you arrive early for your appointment to allow for any paperwork that we may require. When time allows we will mail this paperwork to your home address or you can download and complete these forms from our site. If you arrive late for your appointment it may be necessary to reschedule for another date.

Please bring a list of your current medications to your appointment as well as any recent labs, x-ray results (preferably on CD-ROM) or previous endoscopic procedures done outside of our practice. If you have completed our health questionnaire and demographic forms in advance present them to our receptionist. This allows us to perform a more thorough workup. We will also require a copy of your most current insurance card.

We understand that situations arise in which you must cancel your appointment.  It is therefore requested that if you must cancel you provide us with sufficient notice.  Please see below for the specific requirements for office and endoscopy cancellations.

Office Cancellation Policy:  If an office appointment is not cancelled at least 24 hours in advance, you will be charged a $50 fee.  This will not be covered by your insurance company.

Endoscopy Cancellation Policy:  If your procedure is not cancelled at least 48 hours in advance, you will be charged a $100 fee.  This will not be covered by your insurance company.

No Show Policy:  If you fail to show for your appointment, you will be charged $50 for an office visit, or $100 for an endoscopy.  This will not be covered by your insurance company.

The Cancellation and No Show fees are the sole responsibility of the patient and must be paid in full before the patient’s next appointment can be scheduled.  In addition, if an interpreter is scheduled for your appointment, you may be billed for the interpreter fees which can range from $50 to $300.  These fees will not be covered by your insurance company.

Our practice understands that a good physician/patient relationship is based upon understanding and good communication.  This policy is in place to help us better serve our patients.  Questions about these fees should be directed to the Billing Department at (317) 872-1161.

If you need refills mailed or called in before your next scheduled appointment, please complete the Prescription Refill Request form found here on our web site. If you do not have internet access, please call our office during regular business hours. We will make every effort to refill prescriptions on the day they are received but ask that you allow us two business days. Please check with your pharmacy directly to determine if the refill has been called in. If we are unable to refill your request you will be notified by phone or email. Finally, narcotics and other controlled substances will only be refilled during business hours. We ask that you do not call the on call physician for refills requests.

If your call is regarding a life-threatening emergency please call 911 or proceed to the nearest emergency room. If your call is routine please call during our business hours Monday through Thursday, 9:00 a.m. to 2:00 p.m. and Friday 9:00 a.m. to 12:00 p.m. All phone calls will be returned within one business day and in most cases that same day.

A physician is always reachable by phone for urgent matters. If you call during routine office hours please inform our staff the urgent nature of the call and they will page your physician. If the call occurs when the office is closed it will be forwarded to the exchange that will then contact the on call physician. The physician on call will return your call as soon as possible but it may take up to one hour due to other patient care responsibilities.

Indiana Gastroenterology, Inc. (IGI) contracts with most insurance plans. However, it is your responsibility to contact your insurance provider to check your benefits and confirm we are in your network. Copayment is expected at the time of service. It is important that you present a current card for all services rendered. We will contact your insurance company to determine if precertification or prior authorization is required.

You must also notify us of any insurance changes that may affect payment. For example, on June 12 you schedule testing to be done on August 12, we contact your company on June 15 to verify coverage and authorize the procedure for August 12. On July 1 your company changes carriers or coverage with the same carrier, making the original authorization no longer valid. If you do not notify IGI of changes 72 hours in advance of testing, you may have to pay in full for non-authorized services.

We accept monthly payments on all accounts. No interest or finance charges will be applied. We accept payment in the form of cash, check, money order, Visa or MasterCard. Please call our office to make your credit card payment by phone at (317) 872-1161.
